Putnam County Interagency Transition Team
Mission Statement:
The mission of the Putnam County Interagency Transition Team is to assist
students with disabilities and their families in finding the support services
they need to achieve their personal goals in life.
The team meets on the every month and consists of agencies,
services, teachers, parents and students with disabilities. We are always
looking for interested parents and students to be involved. If you would

This committee was formed in September
1999 with the Project Connect grant. Since that time there has been
active membership from Applied Technology, Corner Drug Store, Exceptional
Student Services, FDLRS, Goodwill Services, Migrant Education, Putnam
Behavioral, Putnam County Schools, St. Johns River Community College,
The ARC of Putnam County, Vocational Rehabilitation and new agencies
joining monthly. We have had active participation students, former
students and parents.

Success stories:
We have had 6 students selected to attend the Youth Forum in Tallahassee
over the last two summers. This summer three more students have
been selected to attend and one is returning as a counselor. This
opportunity came from their involvement in the Putnam County Interagency
Transition Team. |
