Attention Parents/ Guardians/Adult Student:

So that your student will receive all the services they may need
after Graduation and due to a long waiting list, we recommend that you do the following:

1. Call the Department of Children and Families, Adult and Developmental Disabilities: 1-888-665-4955
Ask for a referral specialist. You will need your child’s social security number, name, address, date of birth, etc.

2. You will receive a letter and a collateral release form. Sign it and return it promptly. You will be notified by mail if your child is eligible. If you do not hear from them in a few weeks follow up with a phone call.

3. An intake specialist will call to make an appointment with you. At that time you will make an initial support plan. This does not mean your child will start to receive services. This means your child will be placed at the bottom of the list waiting for funding. You will be notified when funding is available. (the waiting list is around year and half to two years at this time)

4. When funding is made available your child will receive a medical waiver and that coordinator will provide guidance for all the services available.
